OFFICE POLICIES AND PROCEDURES

Please Read Carefully and Keep for Your Records

WELCOME and we look forward to working with you to evaluate your health needs from the perspective of Environmental Medicine. This package gives you information about Dr. Krop’s medical practice which incorporates complementary medicine as well as information about fees. Complementary medicine includes services which are not covered by OHIP. We remind you that this is not a primary care practice and you must have a family doctor for ongoing general care.

PLEASE read the enclosed Information About Dr. Jozef Krop’s Practice carefully. Read the Informed Consent Agreement carefully but do not sign it until you have seen Dr. Krop to discuss it. Complete the enclosed questionaires and bring the entire package with you to your first appointment. Please bring copies of any results of recent blood tests, x-rays, consultation reports, etc. from other physicians.

ALL new patients are required to undergo a basic initial evaluation. This includes a general medical assessment consisting of a medical history, physical examination, routine blood and urine tests (this portion is covered by OHIP). It also includes a comprehensive environmental and nutritional assessment, electrodermal screening and two hour educational session (this portion is not covered by OHIP). The initial evaluation is modified for different age groups. Other tests may be recommended which are not covered by OHIP. Follow up medical visits are covered by OHIP.

HEALTH is not achieved through the efforts of the medical staff alone. You as an individual also have the opportunity to take some responsibility for your own progress in improving your health. Success depends to a great degree upon compliance and the mutual respect and understanding of all involved. If, at any time, concerns arise regarding the treatment, please tell us. We can only help to correct situations we are aware of. We are here to help you and encourage you to take part in your health care. Keep in mind that it probably took time for your health to deteriorate and improvement cannot be expected overnight.

WE TRUST this information package is helpful. If you have any questions please do not hesitate to call our office.

TOBACCO

is a major cause of illness and compounds many health problems. It is a waste of time and expense for all concerned to investigate or treat a patient using the procedures in this practice while smoking continues at home by the patient or family members. Your home should be smoke-free for at least 3 months prior to your visit and testing. Speak to your family doctor about the many successful programs available to help you or your family members quit smoking. As noted in our telephone message, anyone who smokes or lives with a smoker will not be evaluated or treated in this office.

SCENT-FREE

is the policy in our office. Due to sensitivites of patients and staff and to maintain good air qualtiy in the office, please ensure that you or anyone accompanying you DO NOT USE OR BRING ANY OF THE FOLLOWING PRODUCTS ON YOUR APPOINTMENT DAYS. Failure to do so may result in your being unable to see the doctor or to be tested:
